Rhinoplasty

Rhinoplasty is the most common cosmetic procedure performed by Dr. Hoffman at North Fulton ENT, and the goal is to create a nose which is functional as well as aesthetic. This can be done in a very natural way, avoiding some of the unnatural or "operated on" look that is created by less experienced surgeons.

Dr. Hoffman has had extensive training and experience in the procedure and understands the need to create a nose which breathes well and looks aesthetically pleasing. Rhinoplasty can be performed as a single procedure, but it is most often done in conjunction with a nasal septoplasty. Reshaping the nose often requires the use of cartilage grafts, and these are taken from the patient. The rhinoplasty procedure can either be done completely internally, or with a small, nearly invisible incision at the bridge of the nose. At the end of the procedure, tape and often a splint are placed on the nose, and these are typically left in place for a week before their removal.

Brow Lift

The brow lift procedure treats aging of the forehead and eyebrow area, and is an excellent adjunct to upper and lower blepharoplasty, as well as a facelift. During a brow lift procedure, the forehead and eyebrows are raised to a more youthful level, and the muscles that cause wrinkles and frown lines in the forehead are weakened. In this way, a smoother forehead is created and the upper eyelid becomes more open and "less tired-looking."

The brow lift can be done in several ways, and the hairline and individual needs of the patient determine which approach is used. The traditional method involves an incision at or just behind the hairline, with the more recent innovation of an endoscopic approach also being offered. In each case, sutures are removed at about one week post-op, and most patients can resume normal social activities soon thereafter.

Chin Augmentation

This procedure is performed for patients who desire a stronger, more pronounced jawline. Chin augmentation is often done in conjunction with a rhinoplasty, as a recessed chin will make a nose appear larger on profile, and vice-versa.

This is a simple procedure which can be performed under general or local anesthesia. A small incision is made under the chin in a natural skin crease, and a preformed implant is inserted directly onto the bone of the lower jaw. The edges of the implant are tapered to fit the individual's needs, resulting in a natural, smooth contour to the jawline. If there is also excessive fat present, this can be removed at the same time, and cervical liposuction may be added to the procedure as well. The incision is closed with sutures which are left in place for four or five days, and a bandage is worn for support for a variable amount of time as well.

Upper Blepharoplasty (eyelid surgery)

Upper blepharoplasties rejuvenate an aging upper eyelid by removing excessive skin and fat from the upper lid.

This procedure can be done under local or general anesthesia and is an effective way to open up the eyes and reverse a "tired" look. An incision is made in a crease in the upper eyelid, and eyelid skin and a variable amount of underlying fat are removed. The skin is closed with sutures which are removed anywhere from three to five days later, and the thin skin of the upper eyelid heals very quickly. When positioned properly and closed carefully, the incision line is soon very hard to see. There is very little down time from upper blepharoplasty, making this procedure a very popular way to restore a youthful appearance to the eyes.

Lower Blepharoplasty (eyelid surgery)

Lower blepharoplasty can be done as an isolated procedure, but it is most often done in conjunction with an upper blepharoplasty. This procedure reverses the signs of aging by treating excess lower eyelid skin and the bags, which normally become present beneath the eyes with age. As we age, the fibrous tissue, which holds back the fat surrounding the eye loses some of its elasticity.

This process can be accelerated in patients who have allergies or sinus disease. Eventually, excessive lower eyelid skin combines with tissue laxity to produce an eyelid that is seen as "droopy." Restoring a youthful appearance to the lower lid is the purpose of the lower blepharoplasty. This procedure can be performed with no external incisions if there is no need for removal of skin. This is done utilizing an incision inside of the lower eyelid lash line, with removal of fat, done from behind the skin. The incision is closed with an absorbable suture and healing occurs very quickly. In cases where excess lower eyelid skin is present, this can be removed via a small, well-hidden incision, which lies just beneath the lower eyelashes. This incision is carefully closed with sutures that are removed 3-5 days later, and the resulting scar is extremely difficult to detect.
